Health Insurance

NDG offers comprehensive medical, dental, vision, life, and short-term disability insurance programs.

Eligibility: Full-time employee working a minimum of 30 hours/week will become eligible for insurance benefits on the first of the month following 30 days of full-time employment.

Employer Covered Premium: The company will pay 50% of your monthly medical premium. Your spouse and dependents are also eligible to join the company insurance plan, with the full monthly premium being your responsibility.

 

Alera Benefits/Employee Navigator

  • All full-time team members’ have access to Health Insurance Information on the Alera Benefits- Employee Navigator Dashboard.

  • If you are full-time, you will get an email to the personal email address you entered during onboarding explaining different plans and options.

  • On the first of the month following 30 days of full-time employment, and after selections are made- the selected plan will kick in.

  • Selections and changes can only be made during this initial hire eligibility period and during open enrollment in September for new plans in October. (except for any qualifying life events).

Whether it’s navigating the complicated healthcare system when someone is ill or dealing with day-to-day personal issues, Forum offers you a health advocacy service to help. We are a resource designed to give you expert advice and guidance to assist with healthcare and insurance-related issues that so often create frustration and challenges.

The Client Service Advocate can assist with items such as explaining benefits, answering questions, researching costs, choosing the right type of care, and so much more!

COBRA

You and your eligible dependents have the ability to retain your medical, dental, and vision coverage through COBRA after your final date of employment.

COBRA packets will be mailed to your most recent address listed in Employee Navigator/Forum by Benefit Coordinators within 30-45 days from your coverage end date.
